So how do you know if you need to fill out one of these forms?

  1. Did you perform Cardiac Arrest Patient?
  2. Did perform CPR? Either by manual or by auto-pulse/Lucas?
  3. Did you transport the patient? Or were they pronounced in the field?

If you answer Yes to all three of the above questions, you should be filling out a CARES data form on your patient.

In regards to question three- only the transporting agency, fills out the Cares data form.  But as you can see per the data fields, they will need your initial information, regarding the patient and times, to complete the form.


All Mason County EMS which resulted in CPR being performed should have a CARES form filled out and submitted to Dr. Hoffman
